Developing a sustainable model of physical activity engagement for Senior residents at Cesar Chavez Foundation facilities through Tivity Health's SilverSneaker program. Thorough subjective and objective assessment of client is performed for understanding as well as baseline measures.
Following rapport building, a time management assessment is filled out. This is for client awareness of schedule, in order to eliminate the common misconception of lack of time. Once SMART goals have been established with client, a complete analysis is performed and the establishment of the program is made using the NASM OPT model.
